Day and evening appointments are available. Please contact our office to schedule. Initial evaluations will take approximately one hour.  Please attempt to be 15 minutes early for the first appointment and remember to bring your physician's prescription and appropriate insurance information. You may print, fill out and bring with you the necessary initial forms, located on this site, before you come in to the office. Loose fitting, comfortable clothing is encouraged.  Kindly give 24 hours notice if unable to keep appointment.
