John Sweeney PT, DPT, OCS has direct access privileges.  Some clients may initiate physical therapy services without a referral from a licensed physician, dentist, podiatrist, or nurse practitioner.  Please contact our office to learn the details of this program.  Periodic reassessments by your physician may be necessary.  Evaluation findings, plan of care, and goals of treatment are forwarded to the referring physician as well as ongoing progress notes and a final discharge summary.  The office now utilizes an electronic medical record.
